Because Mettaton-mun and I thought it would be funny, Bill actually can't read the robot's mind with pinpoint accuracy and is only 90% sure they're even a ghost, so we can keep the secret of those two being cousins however long you like.
It's funny Napstablook has the same fear about going back to a broken world because Bill has been using that exact ideal to gather people to his side. He wants to steal Wonderland from the Queen to build a BETTER place here; where people don't HAVE to go home. Of course given that Bill is a crazy psychopath BETTER is VERY subjective.
I'm not sure how well he'll warm up to Napsta just yet. He's either about to be very pissy or amused by their antics. I guess only time will tell XD
